docs(skill-creator): support profile-specific skill creation
Update skill-creator instructions to: - Default to profile directory when running under a profile - Fall back to global directory when no profile is active - Show both directory structures - Add profile skill removal instructions Co-Authored-By: Claude Opus 4.5 <noreply@anthropic.com>
